Dec.11.2018 | ‘Time is brain’ in stroke treatment, every minute of delay to treatment is said to cost a patient 1.9 million brain cells. When a clot blocks your artery at the start of a stroke, the damage is swift. Out-of-hospital emergency medical services (EMS) not only have to beat the times but are under great pressure from the increased number of stroke victims within a given region.

     Last week on December 4th, during City International Hospital's official launch ceremony of a new 115 EMS-Based Satellite Station, A/Prof. Dr. Tang Chi Thuong – Deputy Director at Ho Chi Minh Department of Health shared “It’s been estimated an annual incidence of stroke in Vietnam at 200,000, leading to over 100,000 deaths. Almost half of all stroke deaths occur in the out-of-hospital environment. Saigon is one of the spots with the high incidence of stroke patients along with other causes of sudden death, automobile accidents; and industrial accidents are putting burdens on external Emergency Medical Services (EMS).“

The EMS system has an important role in the management of the acute stroke patient.

      It is estimated an annual incidence of stroke at 60,000 being accepted. Dr. Tran Chi Cuong, President of Ho Chi Minh Society of Vascular and Interventional Neurology, explained: “Every minute of delay to treatment is said to cost a patient 1.9 million brain cells. When a clot blocks your artery at the start of a stroke, the damage is swift. These advances in acute stroke therapy underlie the concept that ‘time is brain’ and that urgent intervention can limit cerebral damage and increase the chances of survival for “golden hour” patients arriving within 4 or 5 hours. However, many of stroke victims are ineligible due to delayed hospital arrival, little to no chance of survival, neurological deficit and mobility impairment....

     There are now 13 million people living in Saigon (HCMC) making it the Vietnam’s largest and economic hub, and its rapidly rising population has a knock-on effect on the city's traffic. In an area of 10 km, emergency services may take several hours just to reach the patients in critical conditions.  Within a given region, the emergency medical services (EMS) system has an important role in the management of the acute stroke patient. Out-of-hospital Emergency medical services have become big challenge.  

     Over the past two years, the Ho Chi Minh Department of has launched operational 115 EMS-Based Satellite Stations, distributed across almost all communities, usually at Hospitals. Currently, the city has 26 operational 115 EMS-Based Satellite Stations equipped with special type of patient transport ambulances by both the public and private healthcare facilities. In addition, the Department of Health is utilizing a unique network of Medical emergency motorcycles (MEM) and professionals to reach patients and emergency scenes where dense populations, traffic and small streets in slow traditional ambulance and quick response vehicles (QRV).

     A/Prof. Dr. Tăng Chi Thuong, EMS impressed with excitement Emergency Medical Services (EMS) Network design has been gradually playing an important role in providing adequate and high-quality medical services for the public to answer as many emergency requests as possible under dynamic and complex conditions. Instead of patients making self-referrals, more and more people trust in dialing the emergency numbers “115”. Once the future of two-wheel EMS transport shows positive outcomes will create a sustainable environment for safer motorcycling and for motorcycling to develop as part of Vietnam transport policy

     Our future expectation in the collaboration between Ho Chi Minh 115 EMS Center and City International Hospital in the new 115 EMS-based satellite station is sure to pay a vital role in the reduction of delayed emergency transport, respiratory and/or cardiac arrests and improve patient outcomes as well as assuring the patient safety and using the patient feedback as a method to improve patient experiences in the hospital.

     In addition, the Department of Health values collaboration in specialty procedures to treat acute and severe medical conditions, such as stroke. The Vascular & Intervention Center at City International Hospital with its ability to recognize early signs of many acute conditions, such as stroke, sepsis, myocardial infarction, cardiovascular and acute vascular diseases and impending respiratory failure. By identifying these conditions early, appropriate interventions and treatments can be implemented within golden hours.
