Clinical application of Blood Salvage used at City International Hospital saved pediatric patient G.B (11 years old) from a deadly liver tumor.

Blood Salvage, also called Auto-transfusion, is a procedure using a device called Cell Saver to collect the blood the patient loses during surgery, cleans the blood and returns the blood back to that patient.

Cell Saver is the most advanced autologous blood recovery system being used in today modern technology,  designed for surgeries like CABG, valve replacement, trauma, orthopedics, transplant, and other procedures where medium- to high-volume blood loss occurs. With the ability to deliver between 50–60% hematocrit and to remove nearly all traces of undesirable components such as free hemoglobin, the Cell Saver System is the standard of care and a critical tool to help avoid unnecessary allogeneic transfusions.  Recently, City International Hospital is applying this method to preserve a patient's life who refuses blood transfusion in major surgery or emergency circumstances.

Clinical application of Blood Salvage

According to A/Prof. Dr. Nguyen Tan Cuong: “The idea of being able to recover a patient’s own blood and put it back into their body is not new.. Auto -transfusion, as it is known, has used to return just the red blood cells, eliminating the platelets needed for clotting and the white cells required to fight infection. That can lead to complications. The technology could also reduce the demand for donated blood.”

He added: “the potential for substantial bleeding in liver surgery is high compared with other operative procedures. In addition, there are high risks for death, acute kidney failure, blood clotting disorder, and the possibility of dialysis. To preserve a patient's life whilst also respecting their wishes in no transfusion, we decided to use Cell Salvage”

The surgical procedure requires continuous collaboration and cross-communication among experienced surgeons, anesthesiologists and many other related medical staff.  Thanks to the advance in Diagnostic Imaging, we have investigated abdominal CT scans, there were abnormal blood vessels, prognosis during bleeding, and chances were we might be excessively losing without performing it to perfection. Our anesthesiologists  must well-prepared such as blood thinning during surgery, lowering blood pressure to help prevent excessive blood loss and use of Cell Saver.

The surgery was a success. The tumor was removed (10 cm in diameter). The patient is now fully recovered and revisited on September 9th 2018 for follow-up examination.

Patient G.B’s Liver Tumor Surgery without Blood Transfusion on September 5 at City International Hospital.

Nguyen.G.B (11- years old , 28 kilograms in weight) living in District 6 Ho Chi Minh City, was diagnosed with liver tumors 6 months ago. Morning September 5, 2018 at City International Hospital, the boy underwent a successful 2.5-hour-liver tumor removal (10 cm in diameter); with one mind-boggling condition requested by his family that religiously he could not receive blood transfusion from any donor (not even from his family members) but himself. Our doctors’ solution was to use Cell Saver (Blood Salvage) which is a device used in surgery to collect the blood the patient loses during your surgery, cleans the blood and returns the blood back to that patient.. September 12th, 2018 the patient got discharged in a much improved condition and very grateful for the care he received during his treatment at City International Hospital.  .

The surgery was a success since we have very well prepared and investigated any abnormal blood vessels, utilized all the best facilities of the hospital. Typically, during anesthesia, the team has given an epidural analgesic medication so that after recovery, the baby will be less painful, recovering sooner. When a part of the liver is removal, our anesthesiologists proactively reduced blood pressure for less blood flow (about 200ml).
